• Medicare number, CSHC/ HCC /Pension number & DVA details (if applicable)

• Ethnicity

• Occupation

• Advance Care Directives

• Past medical history

• Current medical conditions

• Allergies

• Medications/Treatments

• Specialist information

• Social history

 

On some occasions, your personal information may need to be disclosed.

The following reasons could include:

• Referrals to other medical facilities e.g. Radiology/ pathology, specialists, allied health providers

• Billing purposes

• A serious threat to public safety/health

• A serious or imminent threat to a patient’s life, health and/ or safety

• Required under the compulsion of law

 

Outside of these reasons, we will aim to ensure that your records are not disclosed without your prior written or verbal consent. We will ensure that if written or verbal consent is given, that it is documented correctly and that only the information you request to be given is done so.

The team here at Doctors at the Medical Hub will take necessary steps to ensure that you understand:

All our data is electronically stored and is password protected to ensure security of your information, with passwords being changed on a semi- regular basis to avoid data breech. Only those required to access your information will be able to, as we have multiple tiers of access for staff. Our systems are all backed up off site and on a regular basis.
